Required Documentation

To register with Capital you can complete the online registration form. Once you have submitted the application we will send out a Registration Pack which we will need you to complete and return along with the following documentation:

• An up to date Curriculum Vitae
• Two Current References
• Copy of passport  / visa status
• GMC Certificate (Copy)
• CRB (Criminal Records Bureau) check
• Copy of immunisation reports: Hep B, TB, Mumps, Measles, Rubella
